Who We Are
ASCEND Care Coordination is a New York-based care management organization focused on helping individuals with complex medical, behavioral health, and social needs navigate the healthcare system through coordinated, person-centered support.
Our mission is to improve health outcomes by connecting individuals with the healthcare providers, community resources, and support services they need to live healthier, more independent lives.
ASCEND is preparing to provide Health Home Care Management services through partnerships with New York State Lead Health Home organizations.
How We Support Members
Care Coordination
Helping coordinate communication between physicians, specialists, behavioral health providers, hospitals, pharmacies, and community organizations.
Transitional Care
Supporting individuals during transitions following hospitalization or emergency department visits to reduce complications and improve continuity of care.
Community Resource Navigation
Connecting individuals with housing resources, food assistance, transportation, financial benefits, and other community supports.
Whole-Person Care
ASCEND believes successful care management requires looking beyond medical diagnoses alone. Our approach integrates physical health, behavioral health, social determinants of health, community resources, and family and caregiver support.
Every care plan is developed around each individual's unique goals, strengths, and priorities.
